La cirugía de Artroscopia de Rodilla anterior es un procedimiento en el cual, mediante unas pequeñas incisiones en la parte delantera de la rodilla, se puede acceder a la rodilla para reparar lesiones de menisco, ligamento cruzado anterior o realizar una limpieza de lesiones condrales.
A través de la artroscopia, los ortopedistas y cirujanos de rodilla podrán realizar la reconstrucción del ligamento cruzado anterior, usando injertos del paciente o injertos cadavéricos de ligamento cruzado según sea el caso. Posteriormente realizarán unos túneles para pasar el injerto desde la tibia hasta el fémur y fijarán el nuevo ligamento mediante tornillos absorbibles o insumos especiales de fijación en el fémur y la tibia. En caso de lesión del menisco, el ortopedista procederá a realizar la sutura meniscal de ser necesario. Existen varias técnicas para realizar la reparación de los meniscos. El especialista discutirá con usted la alternativa seleccionada de acuerdo a los hallazgos de su resonancia magnética.
En caso de artroscopia por artrosis o para lesiones del cartílago articular (condrales) es posible realizar pequeñas perforaciones o colocación de injertos mediante artroscpia que ayuden al manejo de síntomas de la rodilla.
Para finalizar, los ortopedistas procederán a suturar la piel y dejarán un vendaje blando en la rodilla. Requerirá del uso de muletas por un periodo de tiempo según la intervención que sea realizada y posteriormente requerirá terapia física.

Cirugía de columna vertebral cervical en clínica privada ¿En qué consiste?

Una operación de columna vertebral cervical es un procedimiento mediante el cual, el especialista en cirugía columna realiza una cirugía con el fin de descomprimir y fijar la columna vertebral cervical que presenta alguna enfermedad como conducto cervical estrecho o hernia discal cervical. El objetivo de la operación de columna cervical vía anterior o posterior es descomprimir los elementos neurales de la región cervical, alinear y recuperar la estructura normal de la columna y aliviar el dolor y las molestias en las manos y brazos del paciente, fruto de la compresión nerviosa. La efectividad de la operación de columna vertebral cervical es alta siempre y cuando el paciente cuente con un diagnóstico adecuado y haya intentado un manejo médico con terapia física juiciosa, pérdida de peso y manejo analgésico y antiinflamatorio. En otras palabras, aquellos pacientes que no han mejorado con el manejo médico no quirúrgico se benefician de la cirugía para aliviar el dolor y mejorar su función de sus manos y actividad física. La decisión de llevar a cirugía de columna a un paciente, debe ser acordada entre el especialista de columna, el paciente y su familia.
El tiempo quirúrgico de una operación de columna vertebral cervical puede oscilar de 2 a 6 horas dependiendo el número de cuerpos vertebrales o "niveles" a intervenir así como el abordaje elegido. A mayor número de niveles requeridos, mayor tiempo quirúrgico.

Posterior a la cirugía de columna vertebral, los pacientes deben mantener buenos hábitos de vida, control de peso, estiramientos de columna y actividad física regular.
Cirugía de coluna vertebral cervical o hernia discal cervical, cuidados tras la operación

Luego de una cirugía u operación de columna cervical o hernia discal cervical, el paciente estará en sala de recuperación y hospitalización normalmente durante un día. Durante este periodo podrá sentir molestias de hormigueo en las manos y el cuello, dificultad para tragar, entre otras. Es recomendable caminar tras el primer día de cirugía en el área de recuperación.

En este periodo de observación, se retomará la ingesta de líquidos y alimentos y se manejará el dolor postoperatorio con medicamentos por vía intravenosa. Adicionalmente se administrará manejo antibiótico profiláctico para prevenir infecciones postoperatorias.

Los primeros días deberá estar en casa o en hotel con un reposo relativo y deberá permanecer en la ciudad de Bogotá. Deberá tomar las medicaciones y/o antibióticos prescritos por el doctor Farfán y deberá seguir las instrucciones entregadas. Posterior a un tiempo (15 días) deberá reiniciar la terapia física y podrá retornar a actividades sencillas que no impliquen esfuerzos del cuello.

A medida que pasen los días postoperatorios y que su estado de dolor mejore con la terapia física, podrá retomar actividades físicas y laborales alrededor de seis semanas postoperatorias. Los puntos de cirugía le serán retirados a los 15 días postoperatorios.

Cirugía de columna vertebral cervical o hernia discal cervical ¿cómo es el procedimiento?

Los pacientes internacionales deberán llegar a la ciudad de Bogotá al menos dos días antes de la intervención con el fin de evitar complicaciones secundarias a la altura de la capital.

Posteriormente será valorado por un especialista de anestesia en una valoración preanestésica con laboratorios de sangre la cual deberá ser aprobada.

Llegará a salas de cirugía con un familiar dos horas antes de la intervención con ayuno de 8 horas al menos. Deberá asistir con ropa cómoda y sin objetos de valor.

Una vez el paciente se encuentre en salas de cirugía será anestesiado (anestesia general) y posicionado boca arriba o boca abajo en la mesa quirúrgica (según sea el caso). Posterior a ello el Doctor Farfán y su equipo procederán a limpiar la zona a intervenir (asepsia y antisepsia) y a posicionar el equipo de rayos X (Arco en C) para ubicar el sitio de su cirugía de columna cervical o hernia discal cervical.

Una vez el especialista identifique la zona afectada, procederá a realizar la incisión (según la técnica elegida) para llegar a la hernia discal o la estenosis cervical y proceder a descomprimir el área mediante el uso de microscopio.

Finalizando el procedimiento, tendrá un pequeño apósito de cubrimiento en el área intervenida y será trasladado al área de recuperación en donde será vigilado de cerca.

Herniated Disc Surgery, what are the common risks?

The patient will be in the recovery room for one day following a herniated disc surgery. Walking in the recovery area after the first day of surgery is recommended.

Fluid and food intake will be resumed during this observation period, and intravenous medications will be administered to manage postoperative pain.

The first few days, patient should stay at home or in a hotel room with relative rest. 

After a certain period of time (usually 8 to 15 days), the patient will be able to resume physical therapy and return to simple activities that do not require exertion of the spinal column. 

Approximately six weeks after surgery, patient will be able to resume physical and work activities. 

The surgical stitches will be removed 15 days postoperatively.

 

How is herniated disc surgery conducted in Bogotá, Colombia?

 

It is recommended that international patients arrive to Bogota at least two days prior to the procedure in order to avoid secondary complications resulting from the altitude of the city.

After that, you will undergo a pre-anesthetic assessment with blood tests, which must be approved by the anesthesia specialist.

After fasting for at least eight hours, patient will arrive at the operating room with a family member two hours before the surgery. Comfortable clothes and no valuables are suggested for surgery day.

Once the patient is in the operating room, he/she will be anesthetized (general anesthesia) and positioned face down on the surgical table. Afterwards, Dr. Farfán and his team will proceed to clean the area to be operated (surgical asepsis antisepsis) and to locate the herniated disc with the X-ray equipment (C-arm).

Finally, an incision in the back (according to the chosen technique) will be performed to reach the herniated disc and proceed to remove it.

A small dressing will be placed on the patient’s back at the conclusion of the procedure, and he or she will be transferred to the recovery area for close monitoring.

Normal, Herniated disc surgery is performed on an outpatient basis. Sometimes if the immediate post-operative pain is not well controlled, the patient will requiere an overnight stay in the clinic.
